Admissions Policy and Procedure

FCA does not discriminate based on race, color, religion, sex, handicap, nationality, or ethnic origin.

A Student Application must be submitted with the student’s IEP and any previous ABA Assessment to assure enrollment.

 The following fees must be paid before enrollment:

• Registration

• Books

Parents are required to submit the following documents for enrollment:

• School Entry Health Exam (DH 3040)

• Florida Certification of Immunization (DH 680)/ or Immunization Exemption form

• Medical History Form

• Previous Schools Withdrawal Form

*Parents/Guardians must attend the Parent Orientation once admission has been completed.*
